How to fill:
- Confirm Section weights (Section I = Achievements, Section II = Behaviour). Recommended total = 100%.
- Complete Employee & Appraisal info.
- For each objective/attribute, select a rating (0 = Unsatisfactory … 5 = Excellent) and add comments.
- Click Calculate Score to see weighted totals, percentage and grade.
- Draw signatures (Assessor & Employee) and then export to PDF or Excel.
Use Restore buttons to revert weights or text to the default template whenever needed.

- C (Satisfactory) — 60–74%: Meets expectations, reliable delivery with specific improvement areas.
- D (Needs Improvement) — < 60%: Below expectations; requires structured development plan.
